Crossover. A worn cartridge in a single-lever faucet or a failed mixing valve can let cold water feed backward into the hot supply, producing lukewarm water throughout the property. Quick check: shut the cold supply to the heater and open a hot tap. If water keeps flowing, cold is entering the hot side somewhere in the plumbing.

One fixture only. If the shower is lukewarm and the kitchen tap runs hot, the heater is fine — that's a cartridge, an anti-scald device, or a mixing valve at that fixture.

Demand that has outgrown the tank. A unit sized for two people serving four hasn't failed. That's a capacity conversation, not a repair.
